Alterations in cardiac function immediately following electrical conversion of atrial fibrillation to normal sinus rhythm.

چکیده
SUMMARY An assessment was made of cardiac function by means of right heart catheterization before and immediately after conversion of atrial fibrillation to sinus rhythm in 11 studies on 10 human subjects. Comparable data during exercise were available in five studies. Medical management of patients in this study was identical to that employed routinely for this procedure. After conversion to normal sinus rhythm cardiac output response was significantly increased only during exercise. The relationship of mean right atrial and mean pulmonary wedge pressures to right and left ventricular stroke work was uniformly improved both because of decreases in these pressures and increases in work. Indirect evidence of mitral valvular regurgitation was diminished to absent after conversion. "'A" waves and the pulse pressure of atrial contraction were very small in the right atrial tracings after conversion and these contours were usually inapparent on pulmonary wedge tracings. It is concluded that overall cardiac performance was improved with re-establishment of sinus rhythm and that diminutions in atrial-venous pressure behind each ventricle may be important in explaining symptomatic improvement following electroconversion. Measures of both pressure and flow were required for complete description of the alterations induced by this intervention.

Intravenous Methylprednisolone, a Possible Cause of the Atrial Fibrillation
We are presenting a case illustrating atrial fibrillation (AF) following the use of methylprednisolone in a patient with pelvic and femur fracture. A 48- year- old man with no significant past medical history, was admitted to the emergency department after injury in a car accident. He suffered a multiple bone fracture with chief complaints of pain and shortness of breath. He was transfer...

RECENT STUDIES in the laboratory animal and in man have revived earlier interest in the contribution of atrial contraction to ventricular performance,1-4 and the influence of autonomic tone on displacement of the cardiac pacemaker.5-8 However, the hemodynamic consequences of acute alterations in the synchrony of atrial and ventricular contraction have never been clearly established in normal ma...

The clinical use of direct current shock in the management of cardiac arrhythmias (Lown, Amarasingham, and Neuman, 1962) has been amply confirmed by many other workers (O'Brien, Resnekov, and McDonald, 1964; Oram et al., 1964; Morris et al., 1964b). The length of time that sinus rhythm persists after the electrical conversion of chronic arrhythmias is often disappointingly short, even when quin...

U NTIL recently enthusiasm for conversion of atrial fibrillation to sinus rhythm in patients with rheumatic heart disease has been dampened by the high rate of reversion to atrial fibrillation soon after conversion. Mitral commissurotomy, if followed by a reduction of left atrial pressure and size, offers the theoretical possibility of a reduction in this reversion rate. Successful commissuroto...
متن کاملSpontaneous Conversion of Atrial Fibrillation to Normal Sinus Rhythm Following Recurrent Cerebral Infarctions
Post-stroke atrial fibrillation has been frequently reported especially in the patients with right insular infarct as an evidence of cerebrogenic mechanism affecting on cardiac rhythm. However, conversion to normal sinus rhythm after stroke in patients who had atrial fibrillation has not been reported. A 88-year-old men who had untreated atrial fibrillation was admitted to hospital due to left ...
